I need a form for the insurance company that says Myself and my husband have custody of our grandson. So he can be insured thru our policy. Any help?

    That’s not a simple form to come by, a court or law would have to issue something like that, and the parents would have to give up custody.

    the courts would have to give you custody of the child meaning the mother and father would both have to sign off there right to the child unless you have had there right terminanted already ?

    and if it is just for insurance the mom may be willing it sounds but you have to have both parents sign off in most states… i would look into your local laws before doing anything

    you can have your daughter write a statement that says you are the legal guardian of your grandson and have it notarized. My grandmother had custody of me and that’s what my mom did and I was able to be on my grandmother’s insurance because of it. Good Luck!

    Social services should be able to help you or be able to tell you where to go. You will need legal gaurdianship papers.
